Vision
Our vision is to build on people’s own capacity to achieve health and well-being for themselves, their families and their communities. We are committed to a continuum of care model that is accessible, equitable, appropriate, holistic and transformative.
Mission
Our mission is to provide, or facilitate access to, a continuum of care - including comprehensive primary health care, illness prevention, health promotion, and community capacity development - for people in the downtown Kitchener, and those from different ethno-cultural groups, with particular emphasis on those who experience barriers to accessing health resources.
Values
We value community through:
- Respect
- Excellence
- Partnership
- Accessibility
- Integration
- Diversity
Approach to the Work
The Kitchener Downtown Community Health Centre believes:
- Health is more than the absence of disease – it’s a resource for daily living.
- Good health depends a many things – the social determinants of health – not just access to health services.
- Health is a shared responsibility.
- People, and communities, know what makes and keeps them healthy.
- Good communication is essential.
- The health of individual people and the communities in which they live are interconnected.
- There is more than one way to work for health and to look at health.
- People and communities should be able to influence the things that make them healthy.
Individual and community health is affected by many things including:
- Education & literacy
- Employment & work environment
- Income
- Housing
- Food access & security
- Environment
- Social support systems
- Social inclusion/exclusion
- Gender
