Kitchener Downtown Community Health Centre

Vision, Mission, Values

Vision

Our vision is to build on people’s own capacity to achieve health and well-being for themselves, their families and their communities. We are committed to a continuum of care model that is accessible, equitable, appropriate, holistic and transformative.

Mission

Our mission is to provide, or facilitate access to, a continuum of care - including comprehensive primary health care, illness prevention, health promotion, and community capacity development - for people in the downtown Kitchener, and those from different ethno-cultural groups, with particular emphasis on those who experience barriers to accessing health resources.

Values

We value community through:

  • Respect
  • Excellence
  • Partnership
  • Accessibility
  • Integration
  • Diversity

Approach to the Work

The Kitchener Downtown Community Health Centre believes:

  1. Health is more than the absence of disease – it’s a resource for daily living.
  2. Good health depends a many things – the social determinants of health – not just access to health services.
  3. Health is a shared responsibility.
  4. People, and communities, know what makes and keeps them healthy.
  5. Good communication is essential.
  6. The health of individual people and the communities in which they live are interconnected.
  7. There is more than one way to work for health and to look at health.
  8. People and communities should be able to influence the things that make them healthy.

Individual and community health is affected by many things including:

  • Education & literacy
  • Employment & work environment
  • Income
  • Housing
  • Food access & security
  • Environment
  • Social support systems
  • Social inclusion/exclusion
  • Gender
