Waterloo Region Diabetes Education Program

Group education and some individual follow-up for anyone in Kitchener-Waterloo who has been diagnosed with Type 2 Diabetes or pre-Diabetes. Referrals are required – contact Grand River Hospital or your primary care provider (doctor, nurse practitioner).
