Substance use has wide ranging effects on people’s lives and our communities. Waterloo Region Integrated Drugs Strategy (WRIDS) is a community change response to address concerns and reduce harms pertaining to substance use through project-based work.

Home and Community Care

 

Community Healthcaring Kitchener-Waterloo is pleased to host a Community Care Coordinator who provides supports regarding questions or potential home care referrals. Our Community Care Coordinator supports include:

 

  • Occupational Therapy

  • Physio Therapy

  • Personal Support Worker

  • SW

  • SLP

  • Nursing (wound care, IV meds; but FYI not general ‘vital sign’ checks/assessments)

  • Nursing Clinic

  • Palliative Care Support

 

House of Friendship – Outreach Worker

 

Family Outreach Workers walk with families to help meet basic needs by problem-solving solutions together, connecting them with services in their community and providing direct support when necessary.

 

ODSP – Navigator

 

Community Healthcaring Kitchener-Waterloo is pleased to host an Ontario Works Caseworker who helps clients navigate social assistance benefits.

 

St. Mary’s General Hospital – Respiratory Therapist

 

Community Healthcaring Kitchener-Waterloo is please to host a Respiratory testing and education as an extension to St. Mary’s Airway Clinic

 

KW Multicultural Centre – Settlement Worker

 

Community Healthcaring Kitchener-Waterloo is pleased to host a Settlement Worker at our Refugee Health location at 310 King Street East.  This worker assists newcomers to Canada by providing information about important documents – from basic financial assistance to complex citizenship/sponsorship processes. She also links refugees with useful community services and assistance to help them develop plans that will make them successful in their new life and home.
