Community Healthcaring Kitchener-Waterloo (the Health Centre) is committed to meeting the needs of people with disabilities in a timely manner, and will do so by preventing and removing barriers to accessibility and meeting requirements under the Accessibility for Ontarians with Disabilities Act (AODA). Assistive Devices

 

Community Healthcaring Kitchener-Waterloo (the Health Centre) is committed to ensure clients and visitors using our services or visiting our site(s) will be able to use their assistive devices within the limits of any applicable health and safety laws or regulations.  The Health Centre staff, board members, students and volunteers will seek to provide assistance, where possible, to clients accessing Health Centre services when their personal assistive device cannot be used. Accessible Communications

 

Community Healthcaring Kitchener-Waterloo (the Health Centre) is committed to communicating effectively with all people. The Health Centre will communicate and provide information regarding its services and facilities in an accessible manner, upon request. Accessibility Standards for Customer Service

 

The Accessibility for Ontarians with Disabilities Act, 2005 (the AODA) is a provincial act with the purpose of developing, implementing, and enforcing accessibility standards in order to achieve accessibility for persons with disabilities with respect to goods, services, facilities, accommodation, employment, buildings, structures, and premises. Community Healthcaring Kitchener-Waterloo (the Health Centre) is committed to providing services in a way that respects the dignity and independence of people with disabilities. Support Persons

 

Community Healthcaring Kitchener-Waterloo (the Health Centre) welcomes people with disabilities who are accompanied by a support person to all areas of our premises unless that area is not open to the public or third parties.  At no time will a person with a disability who is accompanied by a support person be prevented from having access to the support person while on our premises. Community Healthcaring Kitchener-Waterloo is committed to keeping your personal information safe and private.  The Health Centre has a policy that explains how and why we collect and use your personal information. Clients, therefore, will be asked to give consent prior to receiving or participating in any services offered and/or provided by the Health Centre. Eligibility for Primary Care Services

 

The Health Centre is committed to serving individuals who live within Kitchener-Waterloo and who do not have a family physician (provider). The Health Centre is dedicated to serving individuals who have found it difficult to find or keep a Provider, recognizing that many social determinants of health may be a causative factor or barrier for the individual to obtain Primary Care elsewhere. Right to Primary Health Care

 

Community Healthcaring Kitchener-Waterloo (the Health Centre) is committed to provide a supportive environment where LGBTQ2S+ clients have access to respectful, medically appropriate, and culturally competent treatments and care, and programs and services. Board Code of Conduct

 

Community Healthcaring Kitchener-Waterloo (the Health Centre) Board of Directors expects of itself and its members ethical, responsible, respectful, and lawful conduct. This commitment includes the conscientious use of authority, and appropriate individual and group behavior throughout one’s term as board member. The Board Code of Conduct applies to members in meetings and in public, when acting as a representative of the Health Centre. All board members are expected to conduct business in an environment free from discrimination and harassment, and the board of directors will conform to the legislation set forth in the Canadian Human Rights Act (federal) and the Ontario Human Rights Code (provincial).
